Aorto-iliac interventions

• 62 yr old gentleman
• Reformed smoker
• Known hypertensive
• Claudication pain in both lower limbs
• claudication distance of 50 metres
• Absent pulses in both lower limbs

CT angiogram showed short segment occlusion (complete blockage) of infra renal, pre-bifurcation abdominal aorta with severe stenosis (narrowing) of right and moderate stenosis of left common iliac arteries (the main artery of the abdomen and both legs).


FEMORAL ARTERY ACCESS FOR THE PROCEDURE(PIN HOLE SURGERY)

  

Lesion is crossed using a special wire and catheter

  

A small channel is created through the occlusion

  

  

Aortic stent is placed first and completely expanded using balloons

  

Bilateral iliac stents are placed and expanded using balloons



FINAL RESULT (from left to right): CT angiogram showing complete occlusion of aorta and bilateral iliac artery stenosis; Pre stenting angiogram shows recanalised aorta with severe bilateral iliac stenosis; Post angioplasty and stenting - complete recanalization (opening up) of the aorta and iliac arteries
